Health Agreement
Cottonwood
Puppies You will receive a signed the copy with your puppy. You are
referred to as "Buyer" and Beckie Korthuis of Cottonwood Puppies
hereinafter referred to as "Seller" voluntarily enters into and agrees
to all terms delineated in this contract. HEALTH GUARANTEE
I Seller
guarantees that Puppy is in good health and has received appropriate
medical inoculations and care including distemper & parvo
inoculations and worming. Seller will provide Buyer with a documentation
detailing treatment Puppy has received under Seller's care.(Health
record)
II. Seller guarantees that Puppy is free of life threatening
conditions for 72 hours after delivery to Buyer. Seller recommends a
thorough vet exam within this time frame. Should a serious condition be
found during the vet examination Buyer must communicate with the Seller
immediately. Buyer must present to Seller in writing the condition by
the buyer's veterinarian, Seller will provide a replacement puppy as
soon as such a puppy is available or a refund of the purchase price only
with in the 72 hour period (3 days). The replacement puppy shipping
will be paid by the Seller In this case, the Buyer must return the pup
to the Seller immediately with all papers surrendered. Buyer assumes
full responsibility for the canine's health after 72 hours. All
Veterinarian costs are the responsibility of the Buyer. It is Highly
recommended that the buyer purchase Pet insurance on your new puppy.
III. Seller guarantees that for a period of 2 year from the date of
birth, should puppy develop any genetic disease, that would
significantly shorten the lifespan, severely diminish the quality of
life, or death of the Puppy, attested to in writing to Seller by Buyer's
veterinarian, and canine specialist, and/or seller's choice of 2nd
opinion veterinarian, the seller will provide Buyer with a replacement
puppy as soon as such a puppy is available. Replacement shipping costs
will be paid for by Seller ). In this case, the Buyer must return the
puppy to the Seller it will be at the Buyer's expense or make
satisfactory arrangements with the Seller. If the Buyer, does not wish
to have a replacement puppy and keep the dog ... The seller will pay up
to 25% of the adoption price toward medical expenses, but medically
appropriate treatment must be administered. Juvenile cataracts, hernia's
and cherry eye, Luxating Patella ,(Cocidia and Giardia , Worms and etc.
are excluded in this guarantee, as they do not diminish the quality of
the dog's life and are not life threatening. Seller strongly recommends
Canine Insurance on your puppy. The Breeder has taken every reasonable.
Precaution to ensure a healthy puppy; including Its Sire and Dam
themselves are free of hereditary defects as far as known. I do not
guarantee any health related issues or loss of life due to reactions
caused by Human neglect, vaccinations or flea and tick preparations
administered either orally or on the skin surface. I do not guarantee
vaccination reactions - Leptospirosis or Coronavirus can have life
threatening effects and can cause liver damage
IV. Buyer agrees to
provide appropriate veterinarian treatment to puppy, including
appropriate inoculations, parasite treatment, and heartworm treatment as
recommended by buyers vet. Buyer also agrees to feed a high quality
food to this puppy. This puppy is sold as a companion pet to be used for
tasks appropriate to the breed. There are no guarantees given as to
this dog's eventual size, breeding ability ,color, coat, teeth or
conformation when he/she reaches maturity. The Buyer agrees puppy will
reside in the home of the Buyer and will not be chained or tied out.
This puppy is being sold with the understanding it is going to a good
home and will become part of the family .
GENERAL The buyer agrees that
he/she Is not acting as or In part of any representative In the purchase
of this The puppy and that, the Buyer will not sell the puppy or any of
its offspring, whether accidentally or deliberately produced to any
representative of a puppy mill ,laboratory, animal broker or pet store.
In the event that the buyer is responsible for the sale or gift of any
of these dogs to the above mentioned organizations, the breeder shall
have recourse in the courts for damages to their names and reputation in
the financial sum of $10,000.00. Seller maintains right of first
refusal. If at any time the buyer is unable to keep the above dog, the
buyer agrees to return the dog to the seller and immediately give the
AKC papers to the seller all transportation costs will be paid by the
buyer. If the buyer, for whatever reason, does not give seller the right
of first refusal, then seller has the right to repossess the dog from
its new owners and seek recourse in the court of law. This Dog / Puppy
will not be surrendered to any Animal Shelter, Humane Society or Rescue
Center It must be returned to the seller at buyers expense.
Should Buyer
choose to break any of the covenants of this Agreement, the remainder
of the Agreement will be considered null and void.

1600 Road, Delta, Colorado, 81416 Accepted methods of payment are Postal
money order, Bank transfer. Master Card and Visa is accepted and paid
at http://shop.cottonwoodfarm.net/ it is a secured site . Payment must
be made 3 Days before shipping, delivery or pick up. In the event a
deposit was made and there is not a puppy available due to an
unpredicted circumstance, then the deposit will be refunded. If
Breeder/Seller has not received balance due for the puppy by 8 weeks of
age the deposit will be forfeited and puppy will be sold to someone
else, unless a prior agreement was reached between Buyer and Seller. If a
deposit was received for a puppy and if for any reason the
Breeder/Seller deems it is in the best interest of the puppy not to be
placed with the Buyer, the deposit will be refunded. Puppy does not
become property of Buyer until puppy leaves Seller’s premise. If for any
reason legal action is to be taken on either side, all legal
proceedings and litigation must take place in the State of Colorado,
County Delta Buyer and Seller agree that this Agreement will be governed
by the laws of the State of Colorado in the city of Delta any part of
this agreement may not be altered, modified or rescinded unless agreed
to in writing by Buyer and Seller. The undersigned agree to uphold all
